Requires the commissioner to develop and issue identification cards for individuals with developmental disabilities upon request by the person, their guardian, or caregiver.

Card front must display the bearer's name, address, date of birth, and a statement explaining the person has a developmental disability and may need assistance understanding directions or may become agitated with certain interactions.

Card back may include an emergency contact name and phone number at the bearer's discretion, plus additional space for information the bearer chooses to add.

Issuance is subject to eligibility and documentation criteria established by the office, with a fee to be determined by the commissioner.

Effective 180 days after becoming law, with immediate authorization for regulations needed to implement the program.
