Establishes a "behavioral health and long-term care plan" to be developed and monitored by the commissioners of mental health, health, alcoholism and substance abuse services, and the director of the office for the aging.

Plan shall coordinate provider services and referrals for individuals with mental illness and/or chemical dependence who need long-term care, excluding dementia cases.

Plan must include recommendations for integrating federal, state, and local funding; facilitating research on best practices; expanding alternatives to nursing homes; and providing support services for family caregivers in community settings.

Plan shall specify implementation actions, the number of people to be served, measurable implementation phases, and a timeline for achievement.

Commissioner of mental health must submit an initial report by October 1, 2019, and annual reports by October 1, 2020 through 2027 regarding implementation, people served, and measurable outcomes.
