Establishes the New York Access to Home for Heroes Program under Article 29 of the Private Housing Finance Law to provide financial assistance for adapting housing for disabled veterans.

Authorizes the Housing Trust Fund Corporation to enter into contracts with eligible applicants (cities, towns, villages, or not-for-profit corporations with experience in home adaptation) to provide grants for home modifications.

Limits individual contracts to a maximum of $500,000 with a completion period not to exceed three years, extendable for up to two additional one-year periods for good cause.

Restricts eligible applicants to spending no more than 7.5% of contract amounts for administrative costs and caps allocation to any single municipality at 50% of total annual awards.

Requires eligible properties to be primary residences of disabled veterans with household incomes not exceeding 120% of area median income, and prohibits funding improvements that are otherwise required by federal, state, or local law.
