Amends the definition of "tanning facility" to include establishments offering ultraviolet radiation devices regardless of whether a fee is charged.

Prohibits tanning facilities from permitting use of ultraviolet radiation devices by anyone under 18 years of age, eliminating previous provisions allowing 17-18 year-olds with parental consent and prohibiting only those 16 and under.

Requires individuals 18 and older to provide government-issued photographic identification and sign a written acknowledgment that they have read warnings and agree to wear safety goggles before using tanning devices.

Allows tanning facilities to retain signed acknowledgment statements off-premises provided an electronic image or facsimile is readily available to facility staff.

Makes this law exclusive and preempts local ordinances, except local laws imposing stricter restrictions already in effect prior to the law's effective date.
