Establishes an Alzheimer's disease outreach and education program administered by the Office for the Aging in consultation with the Commissioner of Health

Program targets medical and non-medical professionals (physicians, nurses, social workers) and consumers, caregivers, and organizations serving persons with Alzheimer's disease and related dementia

Program shall promote earlier identification of dementia in hospitals, nursing homes, clinics, and assisted living facilities; provide information on community support services including caregiver support groups and respite care; and promote public awareness of Alzheimer's resources

Authorizes development of educational materials in multiple formats, training curricula, public service announcements, and toll-free hotlines; materials to be available free or for a fee

Director may appoint advisory councils with representation from healthcare professionals, senior service providers, patient organizations, and consumers; councils members receive no compensation but reimbursable expenses
