Amends public health law by adding section 2745 to allow integration of individuals with traumatic brain injuries sustained after age 21 into mental hygiene programs in counties lacking appropriate TBI services.

Requires the health commissioner and developmental disabilities commissioner to enter a cooperative agreement to implement services in underserved rural areas.

Mandates covered services include medical, housing, vocational, educational, transportation, social, personal care, family support, day programs, community re-entry, outpatient rehabilitation, and habilitation services.

Designates department of health Medicaid resources as the funding source for services provided under this section.

Takes effect 90 days after enactment, with rule and regulation development authorized to begin immediately upon passage.
