Amends Penal Law Section 220.45 to add an exemption from criminal possession of a hypodermic instrument charges for paramedics, emergency medical technicians, and individuals trained in tactical first aid who possess tension pneumothorax decompression needles during official duties.

Maintains existing exemptions for individuals obtaining hypodermic syringes or needles through New York's syringe exchange programs, pharmacy programs, and medical provider-based expanded syringe access programs under Public Health Law Section 3381.

Preserves the classification of criminal possession of a hypodermic instrument as a Class A misdemeanor for unlawful possession or sale.

Takes effect on November 1 of the year following enactment into law.
