Requires the director and commissioner of health to establish criteria determining which programs qualify as "successful" for inclusion in the database, with ability to consult public, governmental, non-profit, and for-profit agencies.

Makes the database publicly accessible on the office's website with information including contact details, program name, program description, and geographic service area.

Mandates the database be updated at least annually by the office in conjunction with the department of health.

Takes effect one year after enactment, with authorization to adopt necessary rules and regulations by the effective date.
