Requires the New York state broadband office to produce a "look back" report detailing all expenditures from April 13, 2015 through the effective date of the act, covering $500 million from the FY2015-2016 Capital Projects Budget and $170 million from the Federal Communications Commission's Connect America Fund.

Look back report must include total public and private funds committed and expended, location of each investment area with zip+4 codes, planned future investments, and other relevant information deemed appropriate by the broadband office.

Mandates annual reports on expenditures for the previous twelve months using the same criteria as the look back report, beginning one year after the initial report and continuing through January 1, 2021.

Requires both the look back and annual reports to be posted on the broadband office's website and distributed to the Temporary President of the Senate, Speaker of the Assembly, Chair of the Senate Energy and Telecommunications Committee, and Chair of the Assembly Energy Committee.

Takes effect immediately upon enactment, with the initial look back report due no later than 180 days after the act's effective date.
