Prohibits long-term care facilities from discriminating against residents based on sexual orientation, gender identity, expression, or HIV status in admission, discharge, room assignment, restroom access, clothing choices, and medical care decisions.

Requires facilities to use residents' correct names and pronouns, assign transgender residents rooms consistent with their gender identity unless requested otherwise, and allow residents to use restrooms matching their gender identity.

Mandates facilities maintain accurate records of residents' gender identity, correct names, and pronouns; protect personally identifiable information about sexual orientation and transition history; and maintain resident privacy during personal care and medical examinations.

Requires all facility staff working directly with residents to receive cultural competency training on LGBT patients at least every two years, with new hires trained within six months of hire, covering health disparities, legal requirements, and best practices for nondiscriminatory care.

Establishes violations as public health law violations subject to enforcement; allows residents to pursue civil action for injunctive relief and damages against facilities that intentionally violate the bill's provisions.
