Authorizes the Department of Health to provide grants or supplemental reimbursement rates to home care agencies for sepsis prevention, screening, intervention, and education activities on a voluntary basis.

Requires home care agencies to integrate standardized sepsis screening and intervention tools into electronic health records systems that align with hospital sepsis protocols.

Mandates timely exchange of health information for patients meeting sepsis criteria between home care agencies, emergency medical services, hospitals, primary care practitioners, and other clinical partners.

Establishes sepsis collaboratives under the hospital-home care-physician collaboration program to pilot test clinical pathways, care transitions, and readmission avoidance strategies for sepsis survivors.

Requires home care agencies to conduct sepsis education and training for health aides, personal care aides, and family caregivers, and to conduct community outreach with hospitals, physicians, emergency services, and sepsis organizations.

Directs the commissioner to report to the legislature and governor within eighteen months on sepsis-related activities and outcomes achieved under this act.
