New York state Elks Association members may request a distinctive license plate bearing the words "Elks Association" upon proof of active membership.

Applicants must file with the commissioner in a form prescribed by the commissioner and verify their membership annually.

An additional annual service charge of $25 is required for the distinctive plate, in addition to regular registration fees under Vehicle and Traffic Law section 401.

A $6,000 surety bond must be delivered to the Department of Motor Vehicles before plate design and production, unless at least 200 orders with service charges are received beforehand.

If fewer than 200 plates are sold within two years of first becoming available, the department may recover against the surety bond proportionately; all service charges collected are deposited into the distinctive plate development fund for design, production, advertising and distribution costs.
