Requires health insurance policies and contracts covering prescription drugs to include all FDA-approved contraceptive drugs, devices, and products specified in Health Resources and Services Administration (HRSA) guidelines without cost-sharing.

Mandates coverage of contraceptive patient education and counseling without cost-sharing, plus follow-up services including management of side effects, adherence counseling, and device insertion and removal.

Allows insurers to include only one form of contraception per FDA-approved method in their formulary if at least one form is covered without cost-sharing; requires coverage of physician-recommended methods based on medical necessity without cost-sharing.

Permits initial three-month contraceptive supply dispensing with option to dispense up to twelve-month supply for subsequent refills of the same prescribed contraceptive.

Exempts religious employers from contraceptive coverage if they meet specific criteria, but requires written notice to prospective enrollees listing refused services; effective January 1, 2019.
