Mandates creation of a supplemental notification form listing all required notifications to be provided to applicants for public assistance

Requires all notifications on the supplemental form to be printed in at least 16-point font with check-off boxes for applicants to confirm receipt and understanding

Requires local social services districts to provide the supplemental notification form to all public assistance applicants

Directs the Office of Temporary and Disability Assistance to review and update the supplemental form at least every six months to add any new required notifications
