Creates a "Family Bill of Rights and Privileges" for qualified personal representatives (spouses, parents, adult children, adult siblings, legal guardians) of individuals with developmental disabilities receiving services from covered providers licensed or certified by the Office for People with Developmental Disabilities.

Grants family members rights to participate in care plan development, access health and safety information, receive timely notifications of events affecting the individual, visit unannounced, access facility safety plans and food menus, and consult with independent advocates or attorneys.

Requires covered providers to post the bill of rights conspicuously at each facility and allows individuals competent to do so to opt out of specific rights through a written determination recorded in their case file.

Mandates the Commissioner of the Office for People with Developmental Disabilities to post the bill of rights on the agency website, provide translations in English and the top six languages spoken in the state, establish a public education program, and adopt necessary implementing regulations.

Specifies that the bill of rights does not limit other existing legal rights and does not alter appeal rights regarding denial of any listed rights.
