Commissioner of Health shall develop an identification card for people medically diagnosed with Crohn's disease, ulcerative colitis, inflammatory bowel disease, irritable bowel syndrome, or other conditions requiring immediate toilet access

Card may be presented under General Business Law Article 28-F to gain access to restroom facilities

Department shall issue cards upon request with eligibility criteria and documentation requirements established by the commissioner, for a fee determined by the commissioner

Card front must display cardholder's name, address, date of birth, "MEDICAL ALERT: RESTROOM ACCESS REQUIRED," and a statement explaining the condition is non-contagious and requires immediate restroom access

Card back must reference the Crohn's and Colitis Fairness Act and direct complaints to the New York State Attorney General's Consumer Helpline; act takes effect 180 days after enactment
