Requires managed care providers to include written notice of the department of health designated independent consumer assistance program and the independent substance use disorder and mental health ombudsman on all notices of adverse determinations, grievances, and appeals.

Mandates that notices include the name, address, phone number, and website of the consumer assistance program and mental health ombudsman established under section 33.27 of the mental hygiene law.

Applies notice requirements to both managed care providers under Social Services Law section 364-j and health plan organizations under Public Health Law section 4408-a.

Takes effect 180 days after becoming law, except that amendments to section 364-j will be repealed when that section is repealed.
