Amends Mental Hygiene Law Section 29.15 to require inclusion of material about extreme risk protection orders in written service plans for patient discharge or conditional release to the community.

Mandates that informational material explain how patients and authorized representatives may file applications for extreme risk protection orders under Civil Practice Law and Rules Article 63-A.

Requires material to include the factors courts may consider when determining whether grounds exist to issue an extreme risk protection order under CPLR Section 6342(2).

Preserves existing requirement for evaluation of patient's need and eligibility for public benefits including public assistance, Medicaid, and supplemental security income.

Takes effect immediately upon enactment.
