County departments of health serve as the reopening authority for indoor gym and fitness center activities and must review individualized safety plans submitted by health clubs.

Health clubs may only reopen for indoor activities when their region reaches phase four or beyond under executive order 202-20 and maintains a COVID-19 infection rate of no more than five percent.

Only residents of the region where a health club is located, verified by government-issued driver's license or identification, may enter the facility.

All individuals entering the health club must complete a COVID-19 screening questionnaire, have their temperature taken and recorded, and provide a negative COVID-19 test if they answer yes to any screening questions.

Health club operators must sanitize the entire facility every twenty-four hours and ensure guest safety at all times.
