Amends Public Health Law Section 2803-j to require maternity information leaflets include statistics on maternal outcomes and maternal deaths at hospitals and birth centers

Requires annual reporting of maternal deaths from pregnancy and delivery complications (from childbirth through six weeks postpartum) and top six complications by mother's age and race

Mandates disclosure of annual statistics on third trimester fetal losses, stillbirths, and childbirth-related injuries including hemorrhages, tissue damage, organ damage during cesarean birth, and third and fourth degree tearing by mother's age and race

Maintains existing requirements for cesarean section rates, vaginal birth after previous cesarean, midwife deliveries, electronic fetal monitoring, forceps delivery, anesthesia/analgesia usage, labor induction/augmentation, episiotomies, birthing rooms, and rooming-in availability

Takes effect immediately upon enactment
