Establishes a "Reentry for the Elderly Program" under the Correction Law for adults over 50 years old who are under community supervision or will be released from state correctional facilities, using existing department funds.

Requires the Correction Department Commissioner to coordinate with New York City health, mental hygiene, homeless services, and social services departments, as well as county agencies and local stakeholders like senior centers for program implementation.

Program components include geriatric assessment and discharge planning, parole board interview preparation with identification of approved residences, and case management connecting participants to housing, medical/behavioral health care, benefits, employment, and senior center services.

Mandates cooperation from New York City and county health, mental health, and social services commissioners with the Correction Department Commissioner and Office of Minority Health in implementing the program.

Allows the department to contract with non-profit entities to perform program functions, subject to available appropriations, with the act taking effect 180 days after becoming law.
