Adds COVID-19 vaccination to existing immunization requirements for residents and employees of long-term care facilities, alongside influenza and pneumococcal vaccines.

Requires long-term care facilities to document annual COVID-19 immunization for residents and employees, with immunization completed by November 30th of each year.

Directs the commissioner of health to develop a plan prioritizing COVID-19 vaccine distribution to vulnerable populations in residential health care facilities and their employees.

Maintains existing exemptions allowing individuals to refuse vaccines if medically contraindicated, against religious beliefs, or after being informed of health risks.

Requires the commissioner to provide educational materials on COVID-19 vaccination to long-term care facilities and report to state leaders on outbreaks and hospitalizations related to COVID-19 and other diseases.
