Establishes a medication assisted treatment (MAT) program in state correctional facilities administered by the Department of Corrections in conjunction with the Office of Alcoholism and Substance Abuse Services, offering all FDA-approved addiction medications with individualized treatment plans.

Requires reentry planning including information on treatment facilities, housing and employment resources, Medicaid enrollment assistance, and a one-week supply of medication upon release to prevent relapse.

Mandates the Commissioner submit annual reports to the Governor and Legislature analyzing program effectiveness, including impacts on institutional adjustment, behavior infractions, reentry rates, and HIV/hepatitis C treatment outcomes.

Prohibits denying inmates participation based on positive drug screenings or disciplinary infractions, and allows inmates previously receiving MAT to continue treatment during incarceration upon request.

Requires county jails to implement jail-based substance use disorder treatment programs including withdrawal management, all forms of MAT, counseling, peer support, and reentry services, with funding available based on local needs and average daily jail population.
