Requires health care plans and insurers to complete review of health care professional applications within 60 days and notify applicants of credentialing status or need for additional documentation.

Establishes provisional credentialing for newly licensed health care professionals or those relocating to New York who join group practices, allowing them to participate in networks if applications are not approved or denied within 60 days.

Requires the Commissioner of Health and Superintendent of Financial Services, in consultation with stakeholders, to adopt regulations establishing universal application forms, re-credentialing forms, and referral forms for use by all health care plans and insurers.

Mandates that the universal forms be the only forms permitted for credentialing, re-credentialing, and referrals by health care plans, hospitals, and other health care facilities.

Takes effect 180 days after becoming law.
