Amends public health law to establish Article 19 requiring the Department of Health to develop and issue identification cards for individuals with Crohn's disease, ulcerative colitis, inflammatory bowel disease, irritable bowel syndrome, or other medical conditions requiring immediate restroom access.

Cards shall display the holder's name, address, date of birth, a "MEDICAL ALERT: RESTROOM ACCESS REQUIRED" notice, and a statement explaining the conditions are autoimmune, non-contagious, and require immediate restroom access.

Card can be presented under Article 28-F of the General Business Law (Crohn's and Colitis Fairness Act) to gain access to restroom facilities when necessary.

Department shall establish eligibility and documentation criteria for issuance and charge a fee to be determined by the commissioner.

Takes effect 180 days after becoming law, with authority granted to promulgate necessary rules and regulations immediately upon enactment.
