Creates distinctive "Alzheimer's Awareness" license plates available to New York state residents upon request, subject to commissioner approval of design and application procedures.

Charges an additional annual service fee of $25 per plate beyond regular registration fees, deposited to the Department of Motor Vehicles distinctive plate development fund for production, design, advertising and marketing.

Directs annual service charges exceeding $6,000 to the Alzheimer's disease assistance fund established under state finance law.

Requires a $6,000 surety bond before plate production and issuance, waived if the commissioner receives at least 200 pre-orders with fees paid; bond obligations discontinued if 200+ plates sell within two years.

Takes effect 180 days after enactment, except bond requirements take effect immediately.
