Requires managed care providers to include the name, address, phone number, and website of the state-designated independent consumer assistance program and the independent substance use disorder and mental health ombudsman on all notices of adverse determinations, grievances, and appeals.

Mandates that grievance process notices explain the right of enrollees to designate a representative to file a grievance and include contact information for the consumer assistance program and mental health ombudsman.

Requires notices of adverse determinations to include the name, address, phone number, and website of the consumer assistance program and mental health ombudsman established under Mental Hygiene Law section 33.27.

Amends Social Services Law section 364-j and Public Health Law section 4408-a to implement these notification requirements.

Takes effect 180 days after becoming law.
