Adds hunting, fishing, and trapping license applications and renewals as an additional method for individuals to register with the Donate Life Registry for organ, eye, and tissue donations.

Requires the Department of Environmental Conservation to provide space on all hunting, fishing, and trapping license forms for applicants to either register with the Donate Life Registry or decline registration.

Specifies that applicants must be presented with the question "Would you like to be added to the Donate Life Registry?" with options to check "yes" or "skip this question."

Prohibits the Department of Health from maintaining records of individuals who select "skip this question" and clarifies that failing to check a box or declining registration does not imply a wish not to donate.

Takes effect one year after becoming law.
