Requires the New York Department of Health Commissioner to establish an emergency insulin program providing access to analog insulins and related supplies for uninsured or underinsured recipients diagnosed with type one or type two insulin-dependent diabetes by April 1, 2021

Authorizes health care professionals to prescribe and pharmacists to dispense analog insulins through patient-specific or non-patient-specific prescriptions to eligible recipients in emergency situations

Mandates pharmacies with 20 or more state locations and certain healthcare facilities with federal drug discount programs to register as emergency insulin programs and maintain non-patient-specific prescriptions by January 1, 2021

Establishes the Emergency Insulin Program Trust Fund to receive appropriations and contributions from analog insulin manufacturers to support the program, with monies managed by the State Comptroller and Commissioner of Taxation and Finance

Allows the Commissioner to establish a sliding scale cost-sharing arrangement not exceeding $100 for a 30-day supply and requires reporting on program metrics to the Governor and legislative health committees annually in the first year, then biennially
