Requires the Department of Corrections Commissioner, in conjunction with the Office of Alcoholism and Substance Abuse Services, to establish a medication assisted treatment (MAT) program at state correctional facilities for inmates with substance use disorders, including all FDA-approved addiction medications and individualized treatment plans.

Mandates MAT programs provide reentry support including information on treatment facilities, housing, employment resources, Medicaid enrollment assistance, and a one-week supply of medication upon release to prevent relapse.

Establishes standards for county jails to implement equivalent MAT programs and requires the Commissioner to submit annual reports detailing program effectiveness, institutional safety impacts, and participation rates.

Prohibits denying inmates MAT participation based on positive drug screenings or disciplinary infractions, and protects inmates already receiving MAT prior to incarceration to continue treatment throughout their sentence.

Implements a jail-based substance use disorder treatment program requiring withdrawal management, multiple forms of FDA-approved medications, counseling, peer support, discharge planning, and reentry services, with funding allocated based on local needs and jail populations.
