Mandates creation of a supplemental notification form listing all required notifications to applicants, with all text in at least 16-point font.

Requires applicants to check off boxes on the supplemental form indicating receipt and understanding of each notification, and sign the form to confirm they received and understand all notifications provided.

Directs local social services districts to provide all public assistance applicants with the supplemental notification form.

Requires the Office of Temporary and Disability Assistance to review the supplemental form at least every six months and add any new notifications required by law, rule, or regulation.
