Creates a new transition shelter program requiring local social services districts to provide eligible individuals released from incarceration with shelter supplements equal to 85% of fair market rent as established by the federal Department of Housing and Urban Development.

Allows local districts to provide additional supplements up to 100% of fair market rent at their own expense, and requires full coverage of separate fuel for heating expenses.

Provides shelter supplements for up to six months, with extensions available for good cause.

Requires shelter employees to be trained in overdose prevention and to provide housing counseling to eligible individuals.

Makes 100% of costs for shelter supplements, heating expenses, and home stability support services eligible for state reimbursement, using federal funds where applicable and state funds for the remainder.
