Amends the Property Condition Disclosure Statement form required when selling one to four family residential real property in New York to include questions about indoor mold history.

Adds question 19-a to the disclosure form asking whether the property has been tested for indoor mold and requiring attachment of test reports if available.

Adds indoor mold to the list of hazardous or toxic substances that sellers must disclose knowledge of in the environmental section of the disclosure statement.

Takes effect 180 days after becoming law and applies to contracts of sale entered into on or after the effective date.
