Creates Article 31 of the Private Housing Finance Law to establish the New York Access to Home for Heroes Program for disabled veterans facing housing accessibility challenges.

Authorizes the Housing Trust Fund Corporation to enter into contracts with eligible applicants (cities, towns, villages, or nonprofits with disability adaptation experience) to provide grants for home modifications.

Limits individual contracts to a maximum of $500,000 with a completion deadline of three years, extendable for up to two additional one-year periods for good cause.

Restricts eligible applicants from allocating more than 50% of total annual program funds to any single municipality and permits administrative costs up to 7.5% of the contract amount.

Defines eligible disabled veterans as those with honorable or general discharge from active duty in U.S. Armed Forces or National Guard and certified disability, whose primary residence has a household income not exceeding 120% of area median income.
