Commissioner of Corrections, in cooperation with the Commissioner of Motor Vehicles, shall create a department of corrections identification form to be issued to individuals upon release from correctional facilities.

Identification forms must contain the individual's full name, permanent address (if available), birth date, and any other relevant information deemed necessary by the commissioner.

Department of corrections identification form shall constitute sufficient proof of identity for applications for non-driver identification cards and certain driver's licenses, eliminating the need for additional identity documentation.

Acceptable proof of identity for non-commercial driver's licenses and learner's permits is expanded to include the department of corrections identification form, foreign passports, consular identification documents, and foreign driver's licenses.

Act takes effect 180 days after becoming law, with rules and regulations authorized to be implemented immediately upon enactment.
