Establishes a "Reentry for the Elderly Program" under the Correction Law for adults over 50 years of age under community supervision or being released from state correctional facilities, using existing departmental funds.

Requires the Department of Corrections Commissioner to consult with the Office of Minority Health, NYC Department of Health and Mental Hygiene, NYC Department of Homeless Services, and NYC Department of Social Services for implementation in New York City, and with county departments for implementation outside NYC.

Program components include geriatric assessment and discharge planning, parole board interview preparation with identification of approved residences, and case management for housing, medical and behavioral health care, benefits, employment, and senior center services.

Requires cooperation from NYC and county commissioners of health, mental health, and social services, as well as the Office of Minority Health, with the Department of Corrections Commissioner.

Permits the Department to contract with non-profit entities to carry out program functions, subject to available appropriations; takes effect 180 days after becoming law.
