Requires the Department of Environmental Conservation to design combined forms with the State Board of Elections' approval that integrate voter registration requirements with license applications, with costs borne by the Department.

Mandates that voter registration forms include eligibility statements, attestations under penalty of perjury, and explicit information about privacy protections, false application penalties, and the right to decline registration.

Requires the Department of Environmental Conservation to transmit completed registration applications to the appropriate board of elections within ten days of receipt, or by the twentieth day before an election if received between the thirtieth and twenty-fifth day before an election.

Establishes that Department of Environmental Conservation employees must maintain strict neutrality regarding party enrollment, provide equal assistance for voter registration as they do for license applications, and prohibits any discouragement of voter registration; requires the State Board of Elections to develop training programs and oversight rules.
