Mandates that all new forms created after the bill's effective date include two questions: "Have you served in the United States Armed Forces?" and "Has someone in your family served in the United States military?"

Requires staff to inform individuals identifying as veterans or family members about services available through the Division of Veterans' Affairs and local veterans service agencies regarding federal and state benefits.

Restricts use of veteran status information collected under this section solely to referral purposes to the Division of Veterans' Affairs for benefits assistance.

Takes effect on the ninetieth day after becoming law, with authority for agencies to adopt necessary regulations immediately.
