Requires all health insurance policies providing medical, major medical, or comprehensive coverage to cover annual comprehensive medical exams for firefighter insured individuals.

Comprehensive exam coverage must include detailed health history, in-person physical exam covering multiple body systems, vital signs measurement, and specific laboratory assessments including metabolic panel, blood count, lipid panel, thyroid tests, and urinalysis.

Requires coverage for cancer screening services including digital rectal exams, Pap tests, breast exams, colonoscopies starting at age 45, mammograms starting at age 40, and low-dose computed tomography based on risk factors.

Requires coverage for additional testing including spirometry, stress electrocardiogram with maximal oxygen uptake calculation, and chest x-rays every five years, plus any additional testing determined necessary by the health commissioner.

Applies to health insurance policies and HMO contracts issued, renewed, modified, altered or amended on or after the act's effective date; defines "firefighter" as any firefighter regularly employed by a municipal fire department in New York State.
