Establishes the New York Healthy Incentive Program (NYHIP) to provide earned dollars for SNAP recipients to purchase fresh locally grown produce at farm-to-consumer entities and similarly situated stores.

Directs the Department of Social Services to apply for federal grants and waivers from the USDA to implement NYHIP, develop a mobile application and website with an interactive map showing participating vendors, and maintain monthly updates on location and hours information.

Requires the office to automate SNAP benefit cards to allow automatic weekly deductions for community supported agriculture subscriptions and to directly add NYHIP incentives to SNAP cards once federal approval is obtained within 180 days.

Mandates the state hold SNAP beneficiaries and CSA subscription services harmless if benefits are lost during a subscription contract, with the state covering the full cost of the contract.

Establishes a statewide NYHIP outreach program requiring local social services districts to develop annual outreach plans, distribute informational materials through public buildings and media, and provide bilingual resources to inform eligible low-income households about program participation.
