Requires all health insurance policies covering hospital, surgical, or medical care to provide full coverage for annual ovarian cancer testing for high-risk individuals, including FDA-approved tests, transvaginal ultrasound, and pelvic exams, with no deductibles or coinsurance.

Applies requirement to individual policies (Insurance Law §3216), group and blanket policies (§3221), and medical expense indemnity corporations and health service corporations (§4303).

Mandates Medicaid coverage of annual ovarian cancer testing including FDA-approved testing, pelvic exams, transvaginal ultrasound, and other exams for individuals at risk or high-risk regardless of symptoms.

Requires primary care providers (physicians, physician assistants, nurse practitioners, and midwives) to assess patients for ovarian cancer signs and symptoms and offer appropriate testing when indicated.

Directs the Department of Health to develop and distribute informational pamphlets on ovarian cancer signs and symptoms to health care providers, with culturally and linguistically appropriate delivery standards.

Takes effect January 1 following enactment and applies to policies issued, reissued, renewed, modified, or amended on or after that date.
