Amends Social Services Law Section 20 to require the Department of Social Services to establish a centralized homelessness crisis response data system for sharing and accessing information about individuals experiencing chronic homelessness.

System participants include state agencies, local government entities, law enforcement, court systems, school districts, emergency service providers, and other relevant entities designated by the department.

Department must consult with participating entities to identify challenges in addressing chronic homelessness and determine how to improve response strategies through the system.

Department shall enter into data sharing agreements necessary for exchanging information about individuals experiencing chronic homelessness through the centralized system.

System must collect data from other homeless crisis response systems maintained by state entities and aggregate, analyze, and share homelessness information with authorized entities to connect individuals to services and affordable housing.
