Adds health insurance enrollment, renewal, and related transactions as methods to register for organ, eye, and tissue donation through the Donate Life Registry.

Requires space on health insurance forms for eligible individuals to register or decline registration in the Donate Life Registry with a standardized question: "Would you like to be added to the Donate Life Registry? Check box for 'yes' or 'skip this question'."

Applies requirement to forms for health insurance policies covered under Public Health Law section 4408, Insurance Law section 3216, and Insurance Law section 3221.

Authorizes the Commissioner of Health and Superintendent of Financial Services to promulgate rules and regulations immediately for implementation.

Takes effect 180 days after becoming law, or on the same date as Chapter 743 of the Laws of 2021 if that chapter has not yet taken effect.
