Amends the civil practice law and rules to require police officers (as defined in criminal procedure law section 1.20) to file applications for extreme risk protection orders when there is probable cause that a respondent is likely to cause serious harm to themselves or others

Applications must be sworn and submitted with supporting documentation setting forth facts and circumstances justifying the extreme risk protection order

Applications shall be filed in the supreme court in the county where the respondent resides

Application forms must include inquiry about whether the petitioner knows or believes the respondent owns, possesses, or has access to firearms, rifles, or shotguns, with request to list or describe such weapons and their locations

Takes effect immediately upon enactment
