Creates the New York State Public Health Care Option Program to provide comprehensive and affordable health insurance for all state residents who choose to enroll, with coverage competitive in affordability, benefits, and access to quality providers.

Grants the Department of Health Commissioner authority to establish premiums adjusted by household size and income, deductibles, co-payments, and co-insurance while ensuring program viability and affordability, with annual limitations on cost-sharing expenses.

Establishes a 13-member Board of Trustees within the Department of Health comprising the Health Commissioner and Financial Services Superintendent as ex-officio members, plus 5 gubernatorial appointees, 2 Senate appointees, 2 Assembly appointees, and 1 each from Senate and Assembly minority leaders.

Authorizes the Commissioner to establish criteria for provider participation, develop payment methodologies, create enrollment processes for employers with fewer than 100 employees, and seek federal waivers for premium tax credit usage.

Creates a dedicated fund in the joint custody of the Comptroller and Commissioner of Taxation and Finance to receive revenues from premiums, co-insurance, federal payments, and state transfers to support program operations, with implementation plan due within 12 months.
