Defines "health care practitioner" as a physician, nurse practitioner, or physician assistant licensed under New York education law.

Requires health care practitioners treating patients diagnosed with epilepsy at elevated risk for sudden unexpected death in epilepsy (SUDEP) to provide written information about SUDEP risk factors and nonprofit organization contact information.

Directs the Department of Health Commissioner to provide and post written information on the department website to assist practitioners, ensure materials are culturally and linguistically appropriate, and develop guidance for determining whether patients are at elevated risk for SUDEP.

Allows local and national epilepsy organizations to request inclusion of their informational materials and contact information on the department website at the commissioner's discretion.

Provides that health care practitioners found in violation on a first offense shall receive only a warning if the violation was not willful and knowing, with no further civil penalty or discipline.

Takes effect ninety days after becoming law.
