Prohibits long-term care facilities from discriminating against residents based on sexual orientation, gender identity, expression, or HIV status in admission, room assignment, discharge, restroom access, or provision of care.

Requires facilities to use residents' preferred names and pronouns, allow residents to wear clothing and cosmetics of their choice, and protect residents' right to consensual intimate expression unless restrictions are uniformly applied to all residents.

Mandates facilities maintain records of residents' gender identity, correct name, and pronouns; protect personally identifiable information regarding sexual orientation and HIV status; and provide privacy during physical examinations and personal care.

Requires all facility staff working directly with residents receive cultural competency training on LGBT and HIV issues at least every two years, with training covering health disparities, legal requirements, confidentiality best practices, and addressing discrimination and systems of oppression.

Treats violations as violations under existing public health law provisions and allows residents to pursue civil actions for injunctive relief and damages for intentional violations; effective 180 days after enactment.
