Adds new section 4134-a to Public Health Law authorizing the redaction of a physician's name from birth certificates when the attending physician's medical license has been surrendered or revoked by the office of professional medical conduct.

Allows requests for redacted birth certificates to be made by the person whose birth is documented (if 18 or older), their legal representative, or the parent/legal representative of a minor.

Requires the department of health to issue certified copies of birth records with the physician's identity redacted upon receipt of a request in the form or manner to be approved by the department.

Specifies that no court order is required to authorize the redaction of a physician's name under this provision.

Takes effect immediately upon enactment.
