Establishes a two-year short-term aftercare demonstration program for at-risk older adults who have been sheltered at an existing center for elder justice in Bronx County, New York City.

Requires the program to provide at minimum case management services, financial counseling, and mental health services commencing at the conclusion of the discharge process from the shelter facility.

Directs the center to evaluate the program's effectiveness in preventing re-victimization, avoiding readmissions to shelter, avoiding hospitalizations, and maintaining community residence.

Requires submission of findings report to the director, governor, and legislature no later than 180 days after the two-year demonstration program concludes, including potential for statewide expansion and permanent program establishment.

Takes effect 90 days after enactment and expires and is repealed two years after that effective date.
