Amends the hospital-home care-physician collaboration program to explicitly include emergency medical services (EMS) as required partners in collaborative community paramedicine initiatives.

Defines "emergency medical services" to include ambulance services and advanced life support first response services staffed by emergency medical technicians or advanced emergency medical technicians that can provide services beyond initial emergency care and transportation.

Authorizes development of community paramedicine models where hospitals, EMS, physicians, and home care agencies collaborate to provide preventive services to at-risk individuals, reduce avoidable emergency room visits and hospital admissions, and serve individuals with behavioral health or developmental/intellectual disabilities.

Permits emergency medical technicians and advanced emergency medical technicians employed or volunteering with certified ambulance services to participate in community paramedicine programs as part of their work.

Allows the commissioner to provide financing, grants, regulatory waivers, and adjustments to medical assistance rates to support voluntary collaborative initiatives, subject to commissioner approval for state funding or regulatory relief.
